When you complete our online quiz, we collect:
Contact details: first name, last name, email address, phone number with country code, country.
Demographic information: age, sport.
Health-adjacent information you choose to share: the nature of your knee situation (current injury, ongoing pain, lower-extremity issue, or preventative interest), specific injury type (e.g. ACL, meniscus, MCL, LCL, PCL, patella), surgery status and timeline, injury duration, pain location, mobility ("can you walk without crutches?"), specific struggles, and priorities.
This information is collected solely with your consent (the act of completing and submitting the quiz). We use it to:
Generate a personalised assessment via Google Gemini.
Store your details and answers in GoHighLevel so we can follow up with relevant resources, calls, or content.
Match testimonials to your situation when displaying your result.
We do not consider this medical record information under HIPAA and you should not treat it as such — it is self-reported survey data.

6. Data Sharing and Disclosure
We may share your data with service providers:
GoHighLevel (LeadConnector HQ) — primary CRM for quiz leads. Receives every quiz submission including all contact details and quiz answers (situation, sport, injury type, surgery status, surgery/injury timeline, mobility, struggles, priorities, age). Located in the United States.
Google (Gemini API) — AI processing of quiz answers to generate personalised assessments. Receives anonymised quiz answers (not contact info) and a first-name reference for usage tracking. Subject to Google's API data-use terms (not used for model training when accessed via paid API).
Sentry (Functional Software, Inc.) — error monitoring. Receives error metadata (stack traces, browser/runtime info). Personal data is only received if it happens to appear in an error message; the integration is configured to disable session replay.
Vercel — site hosting and first-party traffic analytics (cookieless, no personal data). Server logs may contain IP addresses for short-term operational purposes.
Upstash — managed Redis used for rate-limiting. Holds the visitor's IP address as a counter key for approximately one hour, with no quiz content.
Coaches and internal team members for training purposes
Legal authorities if required to comply with laws
Marketing partners (aggregated and anonymized data only)
Financing partners who assist in payment plan prequalification (e.g., LeadFi)
We do not sell your personal data. We may, however, compile and share anonymized, aggregated statistics with partners.
7. Data Retention
We retain your data:
Quiz lead data (contact details and quiz answers stored in GoHighLevel) is retained until you request deletion, or for a maximum of seven (7) years from the date of your last contact with us — whichever is sooner. This period reflects common accounting and customer-record requirements. Operational data such as rate-limiting counters is retained for no more than one hour.
